8 Brooklyn, PA Brooklyn, PA Full-time Full-time $20 an hour $20 an hour We are seeking a Service Coordinator to join our team! Candidates must live in or near Potter, Clinton, Tioga or Lycoming County ; and comfortable with traveling the neighboring counties.
Service Coordination is the process of working with participants under a waiver program in order to identify, coordinate, and facilitate all necessary health / medical services based on their needs and goals.
You will provide administrative support to clients in either individual or group settings.
Responsibilities:
Assess nature of clients' situations by interviewing and reviewing personal history Develop and execute individual treatment plans Determine appropriate milestones to gauge client progress Communicate client updates to all relevant parties Facilitate referrals to other healthcare professionals and programs Maintain accurate client documentation Meet important deadlines & requirements Travel throughout Pittsburgh & surrounding counties Must have reliable transportation- The SC conducts 4 face to face visits per week.
Must have a valid driver license.
Qualifications:
Bachelor's degree in social work, psychology, human services or related field OR At least 3
years of experience in social service or health care related setting Other Requirements:
Compassionate and caring demeanor Ability to build rapport with clients Strong leadership qualities Excellent written and verbal communication skills Positive attitude Ability to prioritize & meet deadlines We are also looking for bilingual Coordinators ( Spanish, Russian, Chinese, Vietnamese, Korean, etc.
